The application requires a copy of the Ownership Structure Chart, often referred to as the "family tree," illustrating the names of owners, their nationality/jurisdiction, and the percentage of ownership.

• LLC Membership %: The percentage of total LLC membership interests beneficially owned.
• Partnership Allocation %: The percentage of total profit and loss allocations beneficially owned.
